15% off of procedures including:

Extractions, bone grafting, fillings, crowns, bridges, dentures, and partials.

Please review the following details and exclusions carefully:

An enrollment fee is due at signup for all applicable members.
Plan is effective immediately and renews annually on the same date.‍
Membership lasts 12 months (365 days) from purchase.
Only immediate family members in the same household may be added.
This is a Dental Savings Plan, not insurance, and cannot be combined with dental insurance or other offers.
Membership applies only at Weston Family Dental; referred specialists do not accept this plan.
Services only are discounted — retail products (e.g., whitening gel, toothbrushes, Prevident) are excluded.
Bitewing x-rays are 100% covered once per year.
Panoramic/FMX x-rays are covered at 50%, except for new patients, who receive one at 100%.
All membership details are stored in your electronic health record.
48-hour notice is required for cancellations or rescheduling, or the appointment may be forfeited and a fee may apply.
Plan is non-transferable and non-refundable. No substitutions or refunds will be issued if unused.
Rates are subject to change.
Payment is due at time of service.

**Gum health to be determined by doctor. Only applicable to patient's without periodontal gum disease present.
